It seems like the first house points to the general constitution of a person. It's making me think of Ayurveda a bit and how it distinguishes between three doshas or general constitutions and the balance or lack therof of the three. Not just physical constitution but emotional and mental as well. Because it's all interconnected of course.
Im Capricorn rising (no planets) and I'm very pitta. Which results in me having very sensitive skin. Capricorn rules skin. (I think?) When I'm "off" it will show up on my skin. Right now I have a vague rash on my arm and am healing a pimple on my neck (I know gross.) Pluto is also transiting my first house and will be for several more years.

I think I meant Saturn when referring to skin. Thanks for clarification.

Actually I have read on several occations about Saturn and Capricorn ruling over the skin. And I sure had a very nasty cystical acne outbreak just when Saturn was transiting my virgo asc. I had problems for years now even though the cysts are gone. But also Neptune was transiting my 6th house at the same time.

I can understand this, transits trigger, each natal chart has its own layout and is triggered accordingly. My son had acne, natal Saturn conj Asc from 12 and Mars in 1conj Asc, born full moon, Pluto/Uranus conj Moon, those 3 in 6. Sun conj Saturn in 12 and Mars in 1 all opposed Moon in 6. All those planets were in t-square by apex Jupiter in detriment. A very difficult chart. Mars is accidentally dignified in 1 but I've noticed many with Mars in first have or had acne problems.
The Encyclopedia of Medical Astrology by H.L. Cornell, MD...Acne: Inflamation of the Sebaceous Glands from Retained Secretion. A Sun and Aries disease; Sun affl in Aries; Sun sq or opp Moon or Asc at birth, or by direction.; Mars affl in Taurus tends to Acne Rosacea or chronic congestion of the skin of the face; afflctions in the cardinal signs, as such signs denote the skin; Saturn affl in Aries or Taurus, as Saturn tends to retention, and disordered functions of the glands of the skin of the face. (Mentions the involved cervical vertebrae)..., and by afflictions in Aries and Taurus the signs which influence these vertebrae. This disease is also contributed to by disturbed kidneys (then mentions the dorsal vertebrae involved for the kidney place).
Also he writes: "Face: The first house represents the head and face of man. The face is ruled by Mars and is also under the external rulership of the Aries sign. The facial bones are under the structural rulership of Aries. The skin of the face and the complexion are ruled by Venus. The form, shape and characteristics of the face are largely determined by the decanate rising on the ascendant".
Anyway, the natal chart can show what placements lean toward having a propensity for something and the transits show the trigger. The actuality shows through the individual.
