hello,
can Neptune square natal Neptune transit cause fatigue, anxiety and panic attacks? I know its known as mid-life crisis transit but i feel so tired and anxious with no medical cause.

It's possible but it sounds more like Saturn in Pisces. Outer planet to outer planet is generally impersonal.
